*Participants are aware that ballroom dancing, and/or taking ballroom dance lessons, at home or in a studio venue, is a potentially hazardous activity and that there are certain risks of physical injury. Participants agree to assume full risk of any injuries, damages or loss that they may sustain as a result of voluntarily participating in a class or lesson “at home” or at local dance program, including but not limited to falls, physical contact with other participants or the instructor. Participants acknowledge that they are medically able to participate in the activity of ballroom dancing at home or in the studio and do hereby fully release and discharge Ballroom Made Simple LLC, & its representatives from any and all claims from injuries, damages, or loss that may accrue to them while participating in lessons and/or practice time.
